Vance’s ties were further highlighted by a key supporter, former Celgene chairman Bob Hugin, who contributed significantly to a super PAC backing Vance despite facing criticism for cancer drug price hikes during his own failed Senate bid.

Policy Shifts and Populist Positions

Vance’s political record on healthcare has been characterized by a willingness to challenge the GOP establishment, often aligning with populist or even cross-party sentiment, especially on issues affecting his working-class base.

Drug Pricing and Medicare

In 2022, Vance notably signaled support for Democratic efforts to allow Medicare to negotiate drug prices, aligning with the AARP and breaking with the traditional Republican position that opposes such government intervention. This is a crucial indicator of his populist leanings, prioritizing consumers over the pharmaceutical industry. The current 2024 Republican platform, however, avoids heavy detail on policy beyond a promise that Trump «will not cut a single penny» from Medicare and Social Security.

The Opioid Crisis Advocacy

Having witnessed the devastation of the opioid crisis firsthand through his mother’s struggle with drug addiction, Vance has made addressing the crisis a core tenet of his political identity. His perspective frames the epidemic as a symptom of deep-seated economic, social, and cultural despair, advocating for the return of «honorable work» as the solution.

While his initial philanthropic efforts to combat the crisis were short-lived and criticized for ties to a group associated with OxyContin pusher Purdue Pharma, his legislative work in the Senate has focused on practical measures, such as introducing a bill to allow parents to request information on controlled substances prescribed to their adult children on their health insurance.

The Abortion and Transgender Care Tensions

The political spotlight demands alignment with the top of the ticket, which has led to noticeable shifts in Vance’s social policy positions.

Abortion

Vance previously held one of the most uncompromising anti-abortion stances, stating in 2021 that he opposed exceptions for rape and incest. This position clashed with Trump’s current rhetoric, which has moved to leave abortion policy up to the states while emphasizing the need for exceptions in any restrictions. In July, Vance softened his position, publicly supporting a recent Supreme Court decision that preserves access to the abortion pill mifepristone.

This pivot drew sharp criticism from conservative activists. Ben Zeisloft, editor of The Sentinel, expressed his disillusionment on X, suggesting Vance’s shift to support access to the pill was motivated by his pursuit of the Vice Presidential slot and alignment with Trump’s evolving position.

Transgender Healthcare

Conversely, Vance has become a highly visible Senate advocate for restrictions on transgender rights. Last year, he introduced a bill that would make providing gender-affirming care to minors a felony, punishable by 10 to 25 years in prison. The legislation also seeks to prohibit taxpayer funding for gender-affirming procedures for all ages, directly aligning with a priority listed in the RNC platform.

Immigration and Public Health Response

Vance has also taken a firm, anti-immigrant position on health coverage, introducing legislation that would deny federal health coverage to recipients of the Deferred Action for Childhood Arrivals (DACA) program. This mirrors Trump’s rhetoric, which frequently links the influx of undocumented immigrants to the alleged depletion of funds from programs like Medicare.

Furthermore, Vance has been a vocal critic of the federal response to the COVID-19 pandemic, specifically opposing vaccine and mask mandates, which he called an «assault on medical privacy.» His legislative focus on this front includes a bill to prohibit federal mask mandates, which he has characterized as a «failed pandemic policy.»

However, his efforts to hold federal authorities accountable during the East Palestine, Ohio, train derailment demonstrate a commitment to public health advocacy when it affects his constituents. He successfully worked across the aisle with Democratic Senator Sherrod Brown to push for better health monitoring and to urge the Department of Health and Human Services to utilize an Affordable Care Act provision to provide Medicare coverage for residents exposed to the environmental disaster.

Vance’s political identity is clearly shaped by a tension between his venture capital roots in health innovation and his political alignment with populist policies that challenge the very industry he has invested in. As he steps onto the national ticket, his approach to healthcare—a sector critical to the American economy and quality of life—will be a defining feature of the campaign.
